• common indications : trough > 10 mg/l 6 mg / (kg x d)

• for S. aureus septicemia, S. aureus endocarditis, osteoarthritis, … trough > 20 mg/l 12 mg / (kg x d)

• if every other day 18 mg / kg every 48 h
