Why is vancomycin so weak vs MRSA?

• Slowly or poorly cidal, hetero-resistance?

• MIC test is incorrect

• PK/PD “golden target” of a 24h-AUC / MIC of 125 (viz. the fluoroquinolones) is too low… ? should be 400 or even more ???

• Protein Binding of vancomycin, lowering effective AUC ?

• MBC >> MIC for these vancomycin exposed organisms?
